If you are an employee of the University of Tennessee Space Institute, and have an accidental injury while you are working at your job, then you may be covered by the Workers' Compensation Program of the State of Tennessee.

TO FILE A CLAIM so that your medical bills may be paid, you must follow these steps:
  1. REPORT the accident/injury to your supervisor immediately.
  2. SEEK NECESSARY TREATMENT from an authorized  medical provider. The workers' compensation employee handbook and provider directory are now available on the Web. You can also obtain names of authorized physicians from the UTSI Human Resources office or the Sedgwick Claims Management Services, Inc. (See phone numbers below.)
  3. COMPLETE AND SIGN the State of Tennessee "Accident Report Form" immediately and return it to your supervisor. These forms are available in the UTSI Human Resources office or may be filled out online and printed.
  4. YOUR SUPERVISOR will also sign the "Accident Report" and complete a separate "Supervisor's Report", which may also be filled out online." Return these forms to the UTSI Human Resources office as soon as possible.

The UTSI Human Resources office will file your claim for you, and you will be assigned a claims representative by Sedgwick CMS. Your claims representative will be in contact with you to answer questions, pay bills, and help you return SAFELY to work.
